Position Title: Intake Coordinator
Location: Etobicoke
Compensation: $20.00 per hour
Hours: 40 hours per week
Job Description: The Intake Coordinator is responsible for managing all aspects of the patient intake process including collecting appropriate patient information from referral sources, 
establishing and maintaining positive relationships with customers and referral sources, responding to customer requests and concerns, and managing the insurance verification and authorization process.
Demonstrate excellent communication skills with clients, physicians, and marketing reps while conducting all functions related to intake process for Home and Clinic Patients
Review, coordinate, and receive all referrals
Initial communication with various parties with regards to new client referrals Intake of new clients
New referral list tracking
Track deadlines for report submission and follow up with assessors on reports
Communication with assessors for booking requests
Coordination of meetings between various parties
Other communication and coordination of referrals as necessary
Set up patient account information and verify benefits and eligibility
Maintain and updates logs and intake database
Assure that pending referrals are updated with written documentation

Job Requirements: BA in Health Management or Administrative fields
Health Intake experience preferred
Proficient computer literacy including Word and Excel
Must have excellent time management skills to prioritize and organize workload
Positive attitude
Excellent communication skills
